Zoe's MRI Scan--December 18, 2009
Zoe's MRI Results
I am typing this directly as it appears on the results paper the hospital faxed to the pediatrician. Sadly, because I have to work in 40 minutes, I am unable to translate any of it into "human english", lol. The gist of it, that I CAN get, is that she may have a metabolic disease.
Here goes:
FINDINGS
Confluent abnormal signal is seen diffusely involving the suvcortical white matter of both cerebral hemispheres. The deep cortical white matter, basal ganglia, brainstem and cerebellum appear normal in signal. No diffusion restriction or abnormal enhancement is seen. There is no evidence of structural abnormality or space-occupying lesion. The ventricles are not dilated and the subarachnoid spaces are within normal limits for patient's age/ Qualitatively normal MR-spectroscopy shows normal spectrum with no evidence of lactate
The head circumference measures 43cm* in this 2yr8mth old girl, which is below the second percentile.
IMPRESSION
Confluent abnormal signal is seen diffusely involving the subcortical white matter of both cerebral hemispheres. Appearances are suggestive of underlying metabolic disease. The selective diffuse involvement fof the subcortical white matter raises the possibility of L-hydroxyglutaric aciduria however the preservation of the basal ganglia would be atypical for this. Although urea cycle disorder also affect the subcortical white matter, it usually presents in the neonatal period and there is basal ganglia involvement.
In short, Zoe will be returning to Sick Kids Hospital in a few months to to Metabolics Clinic to be tested for a metabolics disorder.
*Although the report given to us read her head circumference to be 44cm, I have adjusted this measurement based on the measurment taken prior to her MRI on the same day and the measurement her pediatrician provided us on January 6, 2010. This is a typo on the report, and it was not the only one.
